SAN MATEO HAUMANA - REGISTRATION
I. POLICY FORM
Registration Fee:
$40 for new students
$40 Annual Registration Fee collected at the beginning of each calendar year.
Tuition:
Tuition is $85 a month (or $25 per class), keep in mind there are normally 4 classes a month.
Holidays will be observed, classes canceled for just cause will not be rescheduled.
For example, classes canceled for competition will not be made up. Payment is due by the first class of the month.
If your payment is late, there is a $5 per week late fee (1-7 days is $5, 8-14 days is $10, past 15 days is $15 max). There is a $25 return check fee.
Class Attendance & Times:
If your child misses a class your tuition does not get reimbursed to you. It is at Kumu’s discretion as to which class your child will be attending. There is a purpose for this.
Performances:
Performances are offered by Kumu. It is at Kumu’s discretion to choose which dancers will participate in performances.
Fundraisers:
Fundraisers are optional and are run by parents.
Attire/Uniform:
Pa’u of appropriate color with KHOOL Shirt.
Hair should be tied back out of the face (ex. Bun, braid, etc)
If your child earns a pa’u that bears the halau print please understand that you will need to purchase this pa’u because it is made specifically for your child, however it belongs to the halau. Therefore, should you stop dancing for an excess of one month this skirt is to be returned with no refunds.
Hula Implements:
Each child needs to have a hula bag.
Please provide a binder for handouts such as words of the song.
Implements are determined by Kumu.
Your child is responsible for not only learning the songs but also doing their own research.
RULES TO REMEMBER
Remember this is OUR hālau, treat it and take care of it as your own. Good examples would be, making sure our shoes and slippers are removed and put away properly. Hula bags are in order with only the items requested inside. If you have taken something out in the waiting room please return. If you have rubbish please throw it away in the rubbish can.
Be prompt and prepared for class. This means on time and ready in your uniform.
Be ready to learn with your mind, body and heart. Hula is an expression of one's inner most feelings.
Understand your Kumu wants you to dance to the best of your ability and will push you by correcting you in the way of our halau that was passed onto her. As a student, please always watch your manners and actions, you are a reflection of not only your parents, but also your Kumu. (And she will find out.)
No negative behaviors or energies in our halau. We want to make sure we have good mana when we learn or make things.
Please remember that Kumu does things a certain way for different reasons that was passed onto her. If you have a question please ask her directly. For example, no recording without permission.

Parents are expected and responsible to participate in any workshops scheduled by Kumu to learn how to create and craft different accessories and aspects of their child's costumes.
Communication is KEY, the door is always open…

III. COVID-19 LIABILITY RELEASE WAIVER
I release Keolalaulani Hālau ‘Ōlapa O Laka, its instructors, and all other associates from liability for harm, theft, or injury that may be suffered by me and/or members of my family traveling to or from or during participation in activities and programs sponsored by Aloha Dalire, Inc. I hereby acknowledge that I am voluntarily assuming full responsibility for all risks of physical injury arising out of active participation in a dance class or other dance related activities. I acknowledge the contagious nature of COVID-19 and other contagious diseases and viruses and voluntarily assume the risk that I and/or my children may be exposed to or infected by COVID-19 by attending and participating and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 and other contagious diseases and viruses may result from the actions, omissions, or negligence of myself and others, including, but not limited to, employees, independent contractors, volunteers, and program participants and their families. I release Keolalaulani Hālau ‘Ōlapa O Laka, its instructors, and all associates from liability for harm, injury or death pertaining to COVID-19 and other contagious diseases and viruses. I understand that there are NO refunds for registration fees, costume, performance fees or classes. I understand that if my dancer’s class gets cancelled due to weather, sickness, or any issues arising from COVID-19, there is no refund for these missed classes. I understand and agree to Keolalaulani Hālau ‘Ōlapa O Laka policies and procedures. I understand that my student must have this release form signed before attending class.
